Transcript Request Form

Students applying for OGS (and/or SSHRC CGS M-Masters) for 2018-19:

OGS applicants are required to upload PDF copies of official transcripts from all post-secondary institutions to the SGS on-line OGS Application.  Because OGS also requires that official transcripts be on file in the Graduate Unit to which students are applying, OGS applicants are responsible for providing these official transcripts to the Graduate English offices.  

However, current Graduate English students have already submitted official transcripts to our Department with their applications for admission to our programs.  It is therefore not necessary for these students to submit additional copies of official transcripts to Graduate English, although these students must still upload PDF scans of official transcripts to the SGS OGS application. 

As their official transcripts are already on file in our offices, current and newly admitted Graduate English students may request PDF copies of their transcripts from Graduate English.
  To request scans of your transcripts for your OGS application, please complete the questionnaire below.  

FOR OGS ONLY:  Note that scans of official transcripts are required from all post-secondary institutions except the University of Toronto; for U of T transcripts for OGS applications,you may create and upload a pdf of a screen capture from ROSI. Do not use on-line digital transcripts or screen captures for SSHRC CGS M, or your application will be ineligible.


FOR SSHRC CGS M - MASTERS APPLICATIONS ONLY:
  Note that scans of official transcripts are required from all post-secondary institutions including the University of Toronto.  If you do not have a scanner, you may order an official  copy of your UofT transcripts from the registrar and we will scan them for you.


IMPORTANT NOTES:
1) OGS transcripts MUST be clearly legible and the legend MUST also be scanned.  If the transcripts are two-sided, both sides must be scanned. Scans and/or screen captures should be uploaded to the OGS application in pdf format. 
2) DO NOT USE A MOBILE PHONE APP TO TAKE PICTURES OF YOUR TRANSCRIPTS. Many mobile phones produce images that are too small or askew, and therefore may not be readable or useable. These images have meant applications were disqualified in the past. If you are not able to make a pdf file of your transcripts, please contact Marguerite Perry at:

The Departmental deadline for you to submit your completed online OGS application is December 8, 2017 (and the SSHRC deadline to submit a SSHRC CGS M - Master's application is before 8pm, December 1, 2017).

Please submit this TRANSCIPT REQUEST FORM a minimum of 2 weeks before your application is due to allow us sufficient time to pull your student records and prepare your pdf scans.  If you have any difficulty using the on-line form, you may make a request by emailing the same information directly to the Graduate Assistant, Marguerite Perry at: marguerite.perry@utoronto.ca
